All new patients will be required to fill out a medical history form when you arrive at the office. If you are taking any medications or health supplements, please bring a list. If you want to prefill the medical history, click here to download the form.

After reviewing your medical history, we will start with a new patient exam that will assess the state of your teeth, gums, supporting bone tissue and soft tissues of the mouth, as well as your jaw joint (TMJ). We will record and analyze this information. This will allow us to estimate potential longevity of your teeth, presence and severity of gum disease, presence of cavities, abscesses, broken teeth, evidence of oral cancer and more. All of this information will help us diagnose any existing problems and come up with an overall treatment plan. Gum exam will help determine how often you need to have your teeth cleaned. You will have the opportunity to ask questions and review the findings with the dentist to make an informed decision regarding any further treatment. We will also be able to give you an estimate of the costs. If time allows, our hygienist will start your cleaning.
